Your Food Diary

If possible, we ask that you keep a written record of all foods that you eat for three days prior to your visit. Be specific. Include the time you ate and the amount that you ate. If you’re seeking nutrition counseling because you’re experiencing symptoms when you eat certain foods, please also record your symptoms before, during and after meals.

Insurance Coverage

We recommend you call your insurance provider before your first appointment to make sure nutrition appointments are covered under your plan.

Ask your insurance provider:

  • Does your insurance plan cover nutrition counseling?
  • If you are covered, do you need a referral from your physician?
  • Are you allowed a certain number of nutrition appointments per calendar year?
Length of Appointment

Initial one-on-one visits last about an hour. We encourage you to come for follow-up visits so we can monitor your progress and answer any new questions you may have. Follow-up appointments usually last between 30–45 minutes.
