Access to your hospital records

Your Medical Records 

You can access a portion of your medical records online any time through our Patient Portal. Your patient portal account includes test results and other details about your medical care.   

At Beth Israel Deaconess Hospital–Needham, the office of Health Information Management maintains patient medical records. You have the right to review or get copies of your medical records for a fee.

How To Request Copies of Medical Records

You can request your medical records by completing the Patient Online Access Form. You will need to provide a valid email address and a government-issued ID. To request copies of a medical record, you must either be the patient or the patient's guardian/legal representative.

If you are unable to complete the electronic form above, download and complete the paper authorization form. Patients and legal guardians may submit their paper authorization form using the following methods:

This form must be legible and complete. Authorizations that are illegible or incomplete will be returned. Please note that processing times are usually faster when requests are made online.

Third-Party Requestors

Health care providers, attorneys or non-patients may request records by uploading a letter of request and a completed authorization form to the Requestor Submission Portal.

How To Request Images

Obtaining your diagnostic images to share with another doctor is easy.

Your images will be added to your electronic medical record after our imaging specialist has reviewed them. If you would like a copy of one of your images, please contact our image archive staff at 781-453-3054. You’ll need to sign a release before receiving your images on a CD.

How to Make a Change to Your Medical Record

If you think something is incorrect or missing from your medical record, you can request an amendment.

To make a change to your medical record, you must complete an amendment form.
